Our patient medical records are set up through a system called digiChart. Before you are seen for your first appointment, you will need to log into DigiChart with your temporary username and password. These will either be given to you when you make your first appointment or they will be mailed to you.

Once you have signed in, you may change your username and password to your own liking. Keep these in a safe place as you will need them each time you update your information. You will be asked to complete personal history, demographics, medical information, allergies and any medications you may be taking. Please note that if your DigiChart information is not completed before your appointment you may be asked to reschedule.

Your prescriptions and radiology orders will be sent through digiChart to your pharmacy or the facility where you will be having radiology done. It is important the correct pharmacies phone number is in our information. If you change pharmacies, please ensure to update your account with the new pharmacy’s number.
