New Patient Information
On this page, you will find all the necessary information required to set up and get you through your first appointment. From here you will be able to set up an appointment, fill out intake forms and set up a new digiChart Patient Portal account. You must set up your digiChart Patient Portal prior to your first appointment.
Our office uses Quest for the majority of our labs. You can download the app that you have access to your labs. The verification process may take a few days.
Prior to your appointment, please send an image of your insurance card, front and back, to email@example.com
If you have an HMO, a referral is required from your PCP. This needs to be sent prior to your appointment, otherwise you may be responsible for the payment of your visit. Some PPO plans also require referrals. Your referral can be sent to us via fax or email.
Family Medical Leave or Disability Forms are completed for you and returned to your employer for a fee of $25.00. Once the requests are received we ask that you give us two weeks to have them completed.
If a wellness form is sent or mailed to the office at a time different than the well exam there is a fee of $25.00 to have it completed.

New Patient Portal
If you already have an appointment and have received details over how to set up your patient portal, you can find the link for that below. Please make sure to set up your account prior to your first appointment. This is required and failure to do so may result in your appointment being rescheduled.
